A. Our staff will ask you to fill out a health history form. These questions are part of the examination and provide us with essential information about you and your health history. Your address, email, and phone number are also very important in case we need to get in touch with you. This information is also required by insurance networks to insure payment reimbursement. All information given to us is confidential.
When it is time to be seen by the physician, the nurse will escort you into and exam room and complete vital signs. Give some thought to your medical history beforehand, such as allergies, symptoms, and current medications. It is helpful to bring medications, or a list of medications to your first visit. The physician will complete an interview and physical exam. A current problem list, medication list, and a plan of care will be determined after your exam. After the visit is complete, be sure to check out at our front desk and schedule any return appointments that are necessary.
