The patient portal is for patients who already see us. In the portal you can:
- Send a message to your care team about a non-urgent question
- See your test results
- View information about your visits
Portal messages are not for urgent problems. If something can't wait, call (513) 860-4600 during office hours. For a life-threatening emergency, call 911. If you need help logging in, call the front desk.
Forms
We have two PDF forms. New patients fill out the New Patient Registration Form before their first visit. Established patients fill out the New Year Paperwork once a year to keep their information up to date. Print the form, complete it and bring it to your visit.

To ask for a refill, call the office or send a message through the patient portal. Your provider needs time to review each request, so please ask before you run out. Some medicines may need a visit before they can be refilled. If you're not sure, call and ask.
Medical records
To ask for a copy of your records, or to have records sent to or from another doctor, call the office at (513) 860-4600. The front desk will explain what's needed.
After hours and emergencies
For a life-threatening emergency, call 911.
