Long-term care from a team that knows you

A chronic condition is a health problem that lasts a long time and needs ongoing attention. Diabetes, high blood pressure, high cholesterol and thyroid conditions are some of the most common. They often cause few symptoms at first, but over time they can affect your heart, kidneys, eyes, nerves and energy.

Managing a chronic condition works best when you see the same practice over time. At Beckett Ridge Family Medicine, we've cared for patients in West Chester Township since 1995. We also care for patients from nearby Liberty Township and Mason. Your provider knows your history, tracks your progress from visit to visit and adjusts your plan as your life and health change.

How we help you manage a chronic condition

  • Regular follow-up. We schedule check-ins based on your condition and how well it's controlled. Some patients come in every few months. Others, once things are stable, come in less often.
  • Lab work as needed. Your provider orders labs to see how your condition and medications are working.
  • Medication management. We review your medications at each visit, look for side effects and interactions, and adjust doses when needed.
  • Lifestyle support. Food, physical activity, sleep, stress and tobacco use all play a role. We help you set goals that are realistic for you.
  • Coordination with specialists. If you need a cardiologist, endocrinologist or other specialist, we help with the referral and stay involved in your care.

High blood pressure

High blood pressure, also called hypertension, often has no symptoms. That's why it's checked at most visits. If your readings are high, your provider may suggest changes in diet, activity and alcohol use, and may prescribe medication. Some patients are asked to check their blood pressure at home and bring a log of readings to their visits. Regular follow-up helps make sure your treatment is working.

High cholesterol

High cholesterol usually causes no symptoms, so it's found through lab work. Your provider looks at your cholesterol along with your other risk factors, such as blood pressure, diabetes, smoking and family history, to understand your overall heart risk. Treatment may include changes in diet and activity, medication, or both, along with follow-up lab work as needed.

Thyroid conditions

Your thyroid helps control your energy and metabolism. An underactive thyroid can cause tiredness, weight gain, feeling cold or dry skin. An overactive thyroid can cause a racing heart, weight loss, shakiness or trouble sleeping. If your provider suspects a thyroid problem, lab work as needed can help find the cause. Many thyroid conditions are managed with medication and regular checks to make sure the dose is right.

Weight, fatigue and brain fog

Weight often plays a role in blood pressure, blood sugar and cholesterol. If you'd like support, ask about our medical weight loss program, which can work alongside your chronic care.

If you still feel exhausted or foggy even when your numbers look good, tell your provider. Fatigue and brain fog can have many causes. Our functional medicine–informed care offers a deeper look when standard care hasn't explained your symptoms.

Getting started

If you've recently been diagnosed, or you're new to the area and need a provider to take over your care, call (513) 860-4600. Ask your previous doctor's office to send your records, and bring your medication list to your first visit.

Step by step

What to expect at your visit

  1. Bring your information

Bring all your medicine bottles or a complete list, any home readings such as blood pressure or blood sugar logs, and recent results from other offices.

  2. Vitals and review

We check your blood pressure, weight and other vital signs and ask how you've been feeling since your last visit.

  3. Talk through your plan

Your provider reviews your results and medications, answers questions and adjusts your treatment if needed.

  4. Labs and referrals as needed

Your provider may order lab work or refer you to a specialist. Results are available in the patient portal.

  5. Schedule your next check-in

Before you leave, you'll know when to come back. Regular follow-up is one of the most important parts of chronic care.
